This clamp which secures the exhaust pipe to the silencer seems to work loose fairly regularly. Am I doing something wrong? Any tips on "doing it properly"?

Try using some anti sieze compound like locktite or never sieze on the thread of the bolt and under the head as well.
You may be putting all your effort into turning the rusty thread instead of making the bolt tight.

Rick's right, exhaust fasteners do suffer some of the most extreme environments and subsequently tend to rust badly. So lubrication the thread means you can feel when the fastener is doing its job rather than just being hard to turn because of the rust.
I use copperslip (you can get little tubes from most motor factors) and you might find using a spring washed will help keep the bolt tight
